Het was een uur of zes in de ochtend toen ze uit het raam sprong. Haar echtgenoot heeft er niets van gemerkt. Of misschien heeft haar echtgenoot het wel gemerkt, maar is hij het daarna spontaan vergeten. Daar heeft hij de laatste tijd vaak last van. Hij vergeet eigenlijk alles onmiddellijk. Hij lijdt aan Alzheimer. Of beter gezegd: zij lijdt aan zijn Alzheimer. Hij is te ver heen om er veel last van te hebben. Maar zij, de moeder van een vriendin, is gebroken. Ze heeft er genoeg van en wil dood. Of ze echt dacht dat ze zelfmoord kon plegen door uit het raam te springen, weet ik niet. In ieder geval is het niet gelukt. Het raam was op de eerste verdieping en ze heeft aan haar val niet meer dan een paar gekneusde ribben overgehouden. Dat mag een klein wonder heten, want ze is al halverwege de tachtig.

Euthanasie

Bij mensen die er een einde aan willen maken denk ik eerder aan getroebleerde jongeren dan aan bedaagde bejaarden. Toch is het merendeel van de mensen die dood willen ziek of bejaard. Uit onderzoek van de psychiater Boudewijn Chabot blijkt dat elk jaar 4400 zieke en oude mensen hun levenseinde in eigen hand nemen. Ongeveer 2800 doen dat door te stoppen met eten en drinken. Nog eens 1600 slikken een dodelijke hoeveelheid medicijnen. Het aantal mensen dat de dood in eigen hand neemt is bijna twee keer zo groot als het aantal mensen dat euthanasie ondergaat.
Over de euthanasiewet die in 2002 is ingevoerd is een heftig politiek debat gevoerd. In de wet staat wanneer een arts een euthanasieverzoek mag inwilligen. Er moet sprake zijn van een duurzame wens en van uitzichtloos en ondraaglijk lijden. De ironie is dat het aantal gevallen van euthanasie sindsdien bijna is gehalveerd. Dat komt enerzijds door de toename van palliatieve sedatie tot bijna 10.000 per jaar. Bij palliatieve sedatie wordt een patiënt in een langdurige slaap gebracht tot de dood intreedt. Maar het komt ook doordat mensen zonder hulp van een arts de dood in eigen hand nemen. De helft van de patiënten die stopt met eten en drinken, heeft een euthanasieverzoek ingediend. Het zijn mensen die zijn gezakt voor hun euthanasie-examen. De arts vond hun ellende niet ernstig genoeg. De andere helft heeft de arts niet willen lastig vallen met hun doodswens.

Een verbod op een overdosis

Het vreemde resultaat is dat er voor een praktijk die steeds minder voorkomt als euthanasie strenge regels gelden, maar dat die regels ontbreken voor de praktijk die ervoor in de plaats is gekomen. Nu kan dat ook moeilijk anders. Je kunt mensen moeilijk bij wet verbieden om een overdosis medicijnen te nemen, te stoppen met eten en drinken of uit het raam te springen. Toch wil je voorkomen dat mensen zo’n besluit nemen omdat ze een depressieve periode doormaken of omdat ze het idee hebben dat ze anderen alleen maar tot last zijn. De beste manier om dat te voorkomen is, paradoxaal genoeg, om de zelfgekozen dood gewoner te maken. Juist door informatie te verstrekken over de verschillende mogelijkheden om er een einde aan te maken, wordt ook de doodwens bespreekbaar.
Boudewijn Chabot heeft nu met een aantal anderen een gids uitgebracht met “Informatie over zorgvuldige levensbeëindiging”. Het is een handleiding voor de dood. Hoeveel heb je nodig van welk medicijn om er zeker van te zijn dat je sterft zonder al te nare bijverschijnselen? Hoe voorkom je dat als je stopt met drinken je mond uitdroogt en er schimmels groeien op je tong en je gehemelte? De tips zijn praktisch van aard en dat is misleidend. Even lijkt het alsof een effectieve en efficiënte dood het hoogste goed is. Niets is minder waar. De tips vormen alleen een hulpmiddel om met dierbaren het scenario van de eigen dood te schrijven. Die samenspraak is essentieel. Je hebt mensen nodig die voorkomen dat je maag wordt leeggepompt als je gevonden wordt voor je sterft. Maar die samenspraak is ook nodig om te voorkomen dat sterven een eenzame onderneming wordt. Want dat is de boodschap van de handleiding: je gaat alleen dood, maar je hoeft niet alleen te sterven.
Als de moeder van die vriendin die gids onder ogen had gekregen, was ze waarschijnlijk nooit uit het raam gesprongen. Ze had over haar doodwens met haar kinderen kunnen praten. Misschien hadden zij het idee uit haar hoofd gepraat, misschien hadden ze samen gekozen voor een dood die bij haar past.
